Ambulatory Care Committee Minutes

Series consists of minutes from the Ambulatory Care Committee and its various subcommittees and/or related committees. Series also includes copies of presentations given to the Committee by various departments within the hospital.

File 7.5.1 includes minutes from the Community Culture Conscious Subcommittee and the House Staff-Liaison Committee. File 7.5.2 includes minutes from the Community Services Subcommittee. File 7.5.3 includes minutes from the Emergency Subcommittee. File 7.5.4 includes minutes from the Executive Committee of the Medical Advisory Board. File 7.5.5 includes minutes from the Community Service Subcommittee. File 7.5.6 includes minutes from the Community Services Subcommittee.

Annual General Meeting minutes

The first Annual General Meeting for the University Health Network occurred on September 15th, 1999. Series includes the contents of a binder labeled “Annual General Meeting Minutes Jun 1947-2005”. This binder included minutes of the University Health Network’s Annual General Meetings (1999-2005) as well as the minutes of the Annual Meetings of the Subscribers of its predecessor corporations The Toronto Hospital (1987-1998) and the Toronto General Hospital (1947-1986). The original purpose of these meetings was to elect Trustees. The annual meeting was extended to several meetings in 1947 and 1949 due to a lack of nominations. Minutes from September 17, 1997 are missing. Most minutes are signed originals.

Archives and Historical Committee

Series consists of minutes, correspondence, historical records and photographs created or collected by the Archives and Historical Committee. Material from the Department of Medical Photography and files from the Public Relations Photo and Slide Library are interspersed within these files. Some of this may be due to the dual roles of Committee members, but in the case of files 2.8.49 to 2.8.57 they appear to be files borrowed by the Chair of the Committee. No attempt has been made to rearrange these files according to their original provenance.

File 2.8.1 includes 2 photographs of Nuclear Cardiology from the early 1980s.

File 2.8.2 contains a photograph of a construction site at TGH in July 1977 and a patient handbook from TGH from [198-?].

Files 2.8.10 to 2.8.13 were originally kept in one binder and probably belonged to Peter Edwards, who was at one point Chairman of the Historical Committee. File names are taken from the divider titles.

File 2.8.16 includes a guide for the della Robia style plaques which hung in the Outpatient Clinical in the Mulock/Larkin Wing of the Toronto General Hospital.

File 2.8.30 includes copies of photographs from the 1940s that were made in 1978.

Audit Committee minutes

Series consists of the draft minutes, agendas, correspondence, and reports of the TTH Board of Trustees Audit Committee from 1989 to 1994. The official minutes for these years have not been transferred to the Archives. File 8.4.5 includes a report from the Office of the Provincial Auditor for The Toronto Hospital - Toronto General Division, 1990-1991. File 8.4.14 includes a report from the Internal Audit Department entitled “Strategic Directions, 1993/94-1995/96.” File 8.4.17 includes 1994 audit reports from the Audit Department; management responses to the audit report and an audit plan and report for 1994.

Canadian Museum of Health and Medicine

Series consists of correspondence and reports from the curator of the Canadian Museum of Health and Medicine, as well as miscellaneous publications accumulated by the Curator. File 2.7.14 to 2.7.17 consists of material donated to the Museum by Dr. John Desmond in October 1997. These materials relate to his privately produced booklets on the history of medicine in Toronto. File 2.7.18 includes a letter from Margaret Best to Dr. Bill Bigelow explaining the history of the photograph of Banting and Best on the roof of the UofT medical sciences building with one of the diabetic dogs (Marjorie?) used in experiments with insulin , and a copy of the booklet TGH 150 Years (1829-1979) with notations.

Caring

Series consists of copies of <i>Caring</i>, the TTH weekly staff newsletter. <i>Caring</i> was first published April 7, 1989, replacing <i>Generally Speaking</i>, the staff newsletter of the Toronto General Hospital (Sub-series 7.1, The Toronto General Hospital fonds).

The newsletter contains news and announcements, including staff appointments and retirements; special events; changes to Hospital services and facilities, organizational charts, obituaries etc.

Files 2.2.8 to 2.2.9 contain some of the master copies of the issues.

There are some errors in issue numbering. Some issues are also missing.

Caring Together

Series consists of copies of Caring Together, The Toronto Hospital’s staff newsletter after the merger between OCI/PMH and TTH. This newsletter combined the news and information formerly reported in the separate publications OCI Post and Caring. The design of the newsletter reflected the new partnership with both logos being combined into one and the colours of the separate hospitals (yellow and blue) seen merging together.

The inaugural issue of Caring Together was first published on September 19, 1997 as Vol 1, Number 1. When The Toronto Hospital changed its name to University Health Network in 1999, Caring Together continued with the same name and volume and issue numbering. The only change was the subheading which now read “A newsletter for and about the staff of University Health Network”. The last issue as Caring Together was Volume 4, Number 45 from November 6, 2000. Beginning with the November 13th issue, the newsletter changed its name to UHN News but continued with the same numbering.

Issues were arranged chronologically by volume in separate binders. Some issues of ITF News, from the Integration Task Force, have been inserted. There are quite a few missing issues of Caring Together. From Volume 2, issues 26, 30, 36 and 37 are missing. From Volume 3, issues 2, 4, 13-17, 19-21, 23, 25, 27, 32, 36, 38, 40, 43-44, and 49 are missing. From Volume 4, issues 6-7, 10, 17-18, 20, 25-30, 33-34, and 43 are missing.

There is no Volume 2, issue 3 in existence. During printing what should have been Volume 2, issue 3 was incorrectly labeled Volume 2, issue 4.

Clinical Quality Assurance Committee minutes

Series consists of the official minutes of the TTH Board of Trustees Clinical Quality Assurance Committee as well as quality management reports presented to the Committee from various TTH departments.

The files were originally stored by year in 3 separate binders. Files are arranged chronologically as per originally binders. Original binder names have been included as headings within the file list.

File 8.5.2 includes a quality management report from the Surgical Directorate, October 1993. File 8.5.3 includes a quality management presentation from the Department of Radiology. File 8.5.4 includes a quality improvement report from the Neurological Centre. File 8.5.5 includes a quality improvement report from the Multi-Organ Transplant Program. File 8.5.7 includes a copy of a quality improvement presentation from the Oncology Program dated May 31, 1994. File 8.5.8 includes a quality improvement report from the Cardiac Centre dated 1994. File 8.5.10 includes a TTH quality improvement status report dated August 1994. File 8.5.11 includes a quality improvement presentation from the Department of Psychology. File 8.5.12 includes a quality report from the Occupational Therapy Department. File 8.5.16 includes a quality report from the Department of Speech-Language Pathology. File 8.5.17 includes a quality report from the Social Work Department. File 8.5.20 includes a quality report from the Department of Clinical Biochemistry. File 8.5.21 includes quality improvement presentations from the Departments of Microbiology and Medical Imaging. File 8.5.22 includes a quality improvement report from the Department of Multi-Organ Transplant. File 8.5.24 includes a quality survey report from the Volunteer Resources Department dated October 1996.
